Which is a common contraindication or precaution for NSAID use?

Active peptic ulcer disease

NSAIDs work by blocking enzymes that make protective prostaglandins in the stomach. Those prostaglandins help maintain the stomach lining and blood flow; when their production drops, the lining becomes more susceptible to irritation, ulcers, and bleeding. So, having an active peptic ulcer means NSAIDs can worsen the ulcer and cause serious GI bleeding, making it a clear contraindication or precaution.

The other conditions aren’t absolute barriers. Controlled hypertension and well-controlled diabetes don’t automatically disqualify NSAID use, though NSAIDs can influence blood pressure and kidney function or affect glucose control in some people, so they’re used with caution. Mild seasonal allergies are generally not a problem with NSAIDs, unless there’s a specific aspirin-exacerbated respiratory condition.
